Sequoia Union High School District Info & Open Enrollment 
For all students who plan on enrolling in Sequoia Union High School District (SUHSD), your "home school" is the one which your student will attend based on where he or she lives.  For SUHSD Enrollment Information, go to www.seq.org.  Please also review SUHSD's letter to families about the Open Enrollment process, which you will use if you wish for your child to attend a high school other than the "home school." The window for Open Enrollment is 11.15.18 - 2.1.19. To determine your home school, call the SUHSD district office at 650.369.1411 x22209.

Student Brag Sheet

Please fill out and email completed Brag Sheet to your student's counselor.

 

Teacher Recommendations

Your student should give teacher recommendation forms DIRECTLY to the individual teachers. If the school you are applying to does not use an on-line application process (such as Ravenna), the student needs to provide a stamped and addressed envelope for each school to which you’d like them sent.

 

Administrator/Counselor Recommendation

Please make sure we receive the electronic request from Ravenna directly to our email address or if the school you are applying does not use an on-line application process, the student needs to provide a paper copy of the recommendation form and a stamped and addressed envelope for each school to which you’d like them sent.

 

Request for Records/Transcripts

This form will come in your Application Packet of the school you are applying. You, the parent or guardian, must sign it and hand in a hard copy to Ms. Ahmad in the front office. If you are applying to more than one school, you may need to sign multiple forms. Records will NOT be released unless we have received a signed authorization for each school.

 

Ravenna: On-line registration.

When applying on-line using the Ravenna application process, please make sure in the Current School section of your Student's Profile that you have selected La Entrada Middle School from their database. 

 

Shadowing / Reporting your Absence

Shadowing is arranged with each individual school. We know this is an important part of the decision making process, but please note, students are responsible for working with their teachers to make up any missed work.  Please report absences in the front office.
